02 · The Anatomy
Four implants, strategically placed.
The genius of All-on-4 lies in the placement angles. The two front implants are placed vertically into the dense bone at the front of the jaw. The two back implants are tilted at approximately 30–45 degrees, anchoring into thicker, denser bone toward the middle of the jaw where natural bone is most plentiful.
This tilted placement avoids the maxillary sinuses in the upper jaw and the inferior alveolar nerve in the lower jaw the two anatomical structures that typically force other clinics to recommend extensive bone grafting.

The science of angled placement.
Tilted implants aren’t a compromise they’re the deliberate engineering solution that makes All-on-4 possible. Here’s the biomechanics behind why this works.
When a tooth is missing for years, the surrounding jaw bone shrinks. This is called resorption, and it accelerates after age 50. By the time most patients consider implants, they’ve lost significant bone height in the back of the jaw typically the last area where teeth fail.
The traditional response to this problem was bone grafting: surgically rebuilding the lost bone with biomaterial, then waiting 6–12 months for it to integrate before placing implants. This adds significant time, cost, and a separate surgical procedure.
The All-on-4 solution
Instead of rebuilding lost bone, tilted implants are placed at angles that seek out the remaining dense bone wherever it is. The back implants tilt forward, finding solid anchorage in the bone that hasn’t resorbed typically the bone closer to the front of the jaw and away from the sinuses.

What healing actually feels like.
All-on-4 healing is largely uneventful. Most patients return to normal eating, working and exercise within 3–7 days of surgery. Here’s exactly what to expect through each phase.
Days 1–3
Immediate post-surgery

All-on-4 is one of the most studied full-arch implant treatments in modern dentistry. Decades of clinical data confirm high success rates but it’s still surgery, and surgery has risks. Here’s an honest look at the real ones.
An individual implant doesn’t integrate.
Occurs in 1–3% of All-on-4 cases over 10 years. Causes: smoking, uncontrolled diabetes, suboptimal initial placement, or low-quality implants.
The fixed bridge cracks or fractures.
Rare with monolithic zirconia bridges (less than 2% in 10 years). Possible with severe untreated grinding or biting on extremely hard objects.

All-on-4 vs All-on-6 — which is right for me?
It depends on bone availability and bite forces. All-on-4 is the right choice when you have moderate to significant bone loss in the back of the jaw, want to avoid bone grafting, or are upgrading from dentures. All-on-6 is preferable when you have sufficient bone, are a heavy grinder, or want maximum long-term stability — particularly for the lower jaw which faces stronger biting forces. Will I really leave with teeth on the same day?
Yes. Every Dentarma All-on-4 case includes same-day fixed temporary teeth attached directly to the four newly placed implants. You go from arrival without teeth (or with failing teeth) to leaving the clinic that same day with a complete fixed bridge. The temporary bridge wears for 3–6 months while bone integrates with the implants, then is replaced with the final permanent zirconia bridge at visit two.
Do I need bone grafting for All-on-4?

How long do All-on-4 implants last?

How do I clean All-on-4 fixed teeth?
All-on-4 fixed teeth need slightly more attention than natural teeth because of the small space between the bridge and the gum. Daily routine includes: regular brushing twice daily, water flosser (Waterpik) once daily to clean underneath the bridge, and an interdental brush as needed. What if my temporary bridge breaks while I'm in the UK?
Rare, but possible. For major repairs, we cover the cost of an emergency return visit to Antalya including flights and hotel. You’re never stranded.
Am I too old for All-on-4?
No upper age limit. We routinely treat patients in their 70s and 80s with All-on-4, often with excellent outcomes because they have stable medical conditions and clear motivations for treatment. The deciding factors are general health and bone quality — not age. Most importantly: many older patients have lived with poorly-fitting dentures for years. How long is the recovery?
Most patients return to normal activity within 3–7 days of surgery. The first 2–3 days involve mild swelling and discomfort manageable with prescribed painkillers. Can I have All-on-4 if I have diabetes or other conditions?
Most chronic conditions are manageable. Well-controlled diabetes (HbA1c below 7.5%), heart conditions on stable medication, and managed blood pressure are not contraindications.
