01 · The Basics
What is All-on-4, exactly?
All-on-4 is a full-arch dental implant treatment that supports a complete set of teeth on just four strategically angled implants — replacing a lifetime of dentures, bridges, or failing teeth with a permanent, fixed solution.
The technique was developed and clinically validated in the early 2000s by Dr. Paulo Maló in Portugal, working alongside the Nobel Biocare research team. The breakthrough was discovering that two strategically tilted implants at the back of the jaw could provide the same support as four straight implants — eliminating the need for bone grafting in the majority of patients.
Twenty years later, All-on-4 has become one of the most widely-performed full-arch implant procedures globally, with over a million cases successfully placed worldwide. At Dentarma, every All-on-4 case is personally placed by Prof. Dr. Emre Şeker — whose PhD in implant biomechanics specifically covers the force distribution patterns that make tilted implants work.
The result is a permanent, fixed bridge of teeth supported by just four implants per arch. You leave the clinic with same-day temporary teeth attached to the implants, return three to six months later for the final zirconia bridge, and walk away with a complete, beautiful smile that lasts decades.
02 · The Anatomy
Four implants, strategically placed.
The genius of All-on-4 lies in the placement angles. The two front implants are placed vertically into the dense bone at the front of the jaw. The two back implants are tilted at approximately 30–45 degrees, anchoring into thicker, denser bone toward the middle of the jaw — where natural bone is most plentiful.
This tilted placement avoids the maxillary sinuses in the upper jaw and the inferior alveolar nerve in the lower jaw — the two anatomical structures that typically force other clinics to recommend extensive bone grafting.
- Front implants — vertical placement in the densest front jaw bone for primary stability.
- Back implants — angled at 30–45° to anchor into thick bone while avoiding sinuses and nerves.
- Same-day fixed bridge attaches across all four implants, supporting up to 14 teeth per arch.
- Force distribution spreads chewing pressure evenly across all four implants — engineered, not improvised.
4
The science of angled placement.
Tilted implants aren’t a compromise — they’re the deliberate engineering solution that makes All-on-4 possible. Here’s the biomechanics behind why this works.
When a tooth is missing for years, the surrounding jaw bone shrinks. This is called resorption, and it accelerates after age 50. By the time most patients consider implants, they’ve lost significant bone height in the back of the jaw — typically the last area where teeth fail.
The traditional response to this problem was bone grafting: surgically rebuilding the lost bone with biomaterial, then waiting 6–12 months for it to integrate before placing implants. This adds significant time, cost, and a separate surgical procedure.
The All-on-4 solution
Instead of rebuilding lost bone, tilted implants are placed at angles that seek out the remaining dense bone wherever it is. The back implants tilt forward, finding solid anchorage in the bone that hasn’t resorbed — typically the bone closer to the front of the jaw and away from the sinuses.
Long-term studies — including the original 10-year, 15-year, and 20-year follow-up trials by Maló Clinics — have shown that tilted implants achieve the same survival rates as straight implants. Approximately 97–99% success at 10 years. The mechanical engineering, when done by an experienced surgeon, is fundamentally sound.
This is why Prof. Dr. Şeker’s PhD in implant biomechanics matters specifically for All-on-4 cases. Knowing exactly where to place each implant — at what angle, at what depth, with what spacing — is the difference between a 20-year success and a five-year failure.
All-on-4 vs the alternatives — which is right for you?

05 · Same-Day Teeth
You arrive without teeth. You leave with teeth.
This is the single most psychologically important feature of All-on-4. Most patients arriving for full-arch treatment have lived for years with failing teeth, ill-fitting dentures, or visible gaps. The fear of “what will I look like during the healing months?” is enormous.
With All-on-4, you never go without teeth. The same surgical appointment that places your four implants also fits a temporary fixed bridge directly onto those implants. You leave the clinic that same day with a complete set of teeth attached to the implants — fixed, eating-capable, looking natural.
The temporary bridge wears comfortably for the 3–6 month healing period while your bone integrates with the implants. When you return for visit two, it’s swapped for the final zirconia bridge — your permanent, durable smile.
From first WhatsApp to final smile.
All-on-4 treatment is split across two visits to Antalya — visit one for surgery and same-day temporary teeth, visit two (3–6 months later) for the final permanent zirconia bridge.

What healing actually feels like.
All-on-4 healing is largely uneventful. Most patients return to normal eating, working and exercise within 3–7 days of surgery. Here’s exactly what to expect through each phase.
Days 1–3
Immediate post-surgery
Mild swelling, light bleeding, and discomfort manageable with prescribed painkillers. Soft food only — soups, smoothies, mashed potato. Most patients describe pain as significantly less than expected.
Days 4–7
Initial soft tissue healing
Swelling subsides, gums begin closing around the implants. Return to softer normal foods. Most patients fly home around day 7 with their temporary fixed teeth working comfortably.
Weeks 2–6
Surface healing complete
Stitches dissolve naturally. Gums fully heal around the implant sites. You can resume all normal activities — work, exercise, social events. Most foods now manageable on the temporary bridge.
Months 1–3
Active osseointegration
Bone cells migrate to the titanium implant surface and form new bone matrix. Implants become increasingly stable. You wear the temporary fixed bridge throughout — fully functional, looking natural.
Months 3–6
Full integration achieved
Bone-to-implant contact reaches maximum stability. Implants are fully osseointegrated and ready to support the permanent zirconia bridge. Time to fly back for visit two.
Are you a good candidate for All-on-4?
All-on-4 is specifically designed for full-arch tooth loss patients — particularly those with significant bone loss who have been told elsewhere they need extensive bone grafting first. If that sounds like you, you’re likely a strong candidate.
- Full or near-full tooth loss
Missing all or most teeth in one or both arches. Including patients still
wearing partial dentures or with several failing teeth ready to be removed.
- Generally good health
Most chronic conditions (controlled diabetes, heart conditions, blood pressure) are manageable. We screen all medical history during consultation.
- Currently wearing dentures
Dentures slipping, painful, or limiting what you can eat. All-on-4 is the most common upgrade path for long-term denture wearers.
- Mature jaw structure
Generally over 18 years of age, with fully developed jaw bones. No upper age limit — we treat patients into their 80s with All-on-4.
- Significant bone loss in back jaw
Told elsewhere you need bone grafting or sinus lift. All-on-4 is specifically designed to avoid these procedures using tilted implant placement.
- Non-smoker or willing to quit
Smoking significantly reduces implant success rates. We strongly recommend stopping at least 2 weeks before surgery.
- Failing teeth across an entire arch
Multiple teeth needing extraction within the same arch. Often more cost-effective to extract all and place All-on-4 than save individual teeth.
- Realistic expectations
All-on-4 is a 4–6 month process from start to finish. The same-day teeth are temporary — the final permanent bridge comes at visit two.
Six reasons UK patients choose All-on-4.
No bone graft needed.
The signature benefit. Tilted back implants avoid the need for bone grafting in 80%+ of patients, even those with significant resorption. Saves 6–12 months of additional treatment time.
You never go without teeth.
Same surgical appointment fits the temporary fixed bridge. You leave the clinic with a complete smile on the same day your implants are placed. No denture-wearing during healing.
Four implants, not eight.
All-on-4 uses just 4 implants per arch vs the 6–8 traditional approach. Less surgical trauma, faster healing, lower cost — without compromising long-term function or success rates.
Long-term data backed.
Developed in 2003 by Dr. Maló in Portugal. 20-year follow-up trials show 97–99% implant survival. Over a million successful cases globally.
Fixed, not removable.
The final zirconia bridge is permanently attached to the implants — never removed by the patient. Eat anything. Speak normally. Sleep with confidence. Brushing and flossing identical to natural teeth.
Half the price of UK private.
Same Straumann Neodent® implants. Same surgical protocol. From £3,400 per arch in Antalya vs £12,000–£15,000 in UK private clinics. The price difference reflects clinic operating costs, not clinical quality.
10 · Risks & Realities
All-on-4 risks, and how we manage them.
All-on-4 is one of the most studied full-arch implant treatments in modern dentistry. Decades of clinical data confirm high success rates — but it’s still surgery, and surgery has risks. Here’s an honest look at the real ones.
An individual implant doesn’t integrate.
Occurs in 1–3% of All-on-4 cases over 10 years. Causes: smoking, uncontrolled diabetes, suboptimal initial placement, or low-quality implants.
The fixed bridge cracks or fractures.
Rare with monolithic zirconia bridges (less than 2% in 10 years). Possible with severe untreated grinding or biting on extremely hard objects.
We use only premium Straumann Neodent® implants placed personally by Prof. Dr. Şeker. Failed implants are replaced free of charge under our lifetime guarantee.
Final bridges made from premium German zirconia (1200 MPa strength). Custom night guard provided to all heavy grinders. 10-year warranty on the bridge.
Risk 03 · Speech Adaptation
Slight speech changes during adaptation.
A common short-term side effect — affects around 30% of patients in the first 2–4 weeks as the tongue adjusts to the new bridge thickness.
Risk 04 · Hygiene Challenge
Cleaning underneath the fixed bridge.
The fixed bridge sits slightly above the gum, creating a small space underneath that requires specific cleaning techniques to prevent peri-implant inflammation over time.
Reading aloud at home for 10 minutes daily resolves it for most patients within 2–4 weeks. Bridge thickness optimised by Prof. Dr. Şeker for individual oral anatomy.
Detailed home care training included. Custom interdental brushes, water flossers, and cleaning protocols tailored to your bridge design. Annual UK check-ups catch hygiene issues early.
Risk 05 · Peri-implantitis
Long-term gum disease around implants.
A long-term risk caused by poor oral hygiene around the implants. Causes bone loss and eventual implant failure if untreated.
“Premium” upgrades or extras added on arrival.
A common Turkey clinic risk — not clinical, but financial. The most common patient complaint in dental tourism.
Annual UK aftercare check-ups in Glasgow, Surrey or North Croydon catch peri-implantitis early. Patient education on home care included with every treatment.

All-on-4 vs All-on-6 — which is right for me?
It depends on bone availability and bite forces. All-on-4 is the right choice when you have moderate to significant bone loss in the back of the jaw, want to avoid bone grafting, or are upgrading from dentures. All-on-6 is preferable when you have sufficient bone, are a heavy grinder, or want maximum long-term stability — particularly for the lower jaw which faces stronger biting forces. We assess your X-ray during free consultation and recommend honestly.
Will I really leave with teeth on the same day?
Yes. Every Dentarma All-on-4 case includes same-day fixed temporary teeth attached directly to the four newly placed implants. You go from arrival without teeth (or with failing teeth) to leaving the clinic that same day with a complete fixed bridge. The temporary bridge wears for 3–6 months while bone integrates with the implants, then is replaced with the final permanent zirconia bridge at visit two.
Do I need bone grafting for All-on-4?
In 80%+ of cases — no. The signature benefit of All-on-4 is that the tilted back implants avoid the areas of the jaw most affected by bone loss. They anchor instead in the dense bone toward the front of the jaw, which retains good volume even after years of tooth loss. This is why patients told elsewhere they “need extensive bone grafting before implants” can usually proceed directly with All-on-4 at Dentarma. We confirm this from your X-ray during free consultation.
Is All-on-4 painful?
Surgery is performed under local anaesthesia, so the procedure itself is painless. Most patients describe post-operative discomfort as significantly less than expected — comparable to a routine extraction or molar removal. Prescribed painkillers handle the first 2–3 days, and most patients are back to normal eating soft food by day 3.
How long do All-on-4 implants last?
All-on-4 has the longest published clinical follow-up of any modern full-arch implant treatment. Long-term studies — including the original Maló Clinics 20-year follow-up — show 97–99% implant survival at 10 years and 90%+ at 20 years. The final zirconia bridge typically lasts 15–25 years before potential replacement. Designed to last a lifetime with proper aftercare.
Can I eat normally with All-on-4?
Yes — once your final permanent bridge is in place, you can eat almost anything you’d eat with natural teeth. Steak, apples, nuts, toast — all manageable. We recommend avoiding biting directly into very hard items (ice, hard sweets, bones) and not using teeth as tools. These rules apply to natural teeth too.
How do I clean All-on-4 fixed teeth?
All-on-4 fixed teeth need slightly more attention than natural teeth because of the small space between the bridge and the gum. Daily routine includes: regular brushing twice daily, water flosser (Waterpik) once daily to clean underneath the bridge, and an interdental brush as needed. Annual UK aftercare check-ups include professional cleaning of all hard-to-reach areas. We provide detailed home care training before you fly home.
What if my temporary bridge breaks while I'm in the UK?
Rare, but possible. If your temporary bridge breaks during the 3–6 month healing period, our UK partner clinics in Glasgow, Surrey, or North Croydon can perform emergency adjustments and minor repairs. For major repairs, we cover the cost of an emergency return visit to Antalya including flights and hotel. You’re never stranded.
Am I too old for All-on-4?
No upper age limit. We routinely treat patients in their 70s and 80s with All-on-4, often with excellent outcomes because they have stable medical conditions and clear motivations for treatment. The deciding factors are general health and bone quality — not age. Most importantly: many older patients have lived with poorly-fitting dentures for years. All-on-4 is genuinely life-changing for them.
How long is the recovery?
Most patients return to normal activity within 3–7 days of surgery. The first 2–3 days involve mild swelling and discomfort manageable with prescribed painkillers. By day 7 you can fly home with comfortable temporary teeth. Soft food only for the first week, then gradually back to normal eating over 4–6 weeks. The full 3–6 month timeline is for bone integration — but you’re functional throughout.
Can I have All-on-4 if I have diabetes or other conditions?
Most chronic conditions are manageable. Well-controlled diabetes (HbA1c below 7.5%), heart conditions on stable medication, and managed blood pressure are not contraindications. We screen full medical history during consultation and may recommend specific pre-operative protocols. Conditions that increase risk: uncontrolled diabetes, recent radiation therapy to the jaw, certain bone-strengthening medications. All discussed honestly during consultation.
What's included in the £3,400 per-arch price?
Everything you need. Four Straumann Neodent® premium implants, same-day temporary fixed bridge, final zirconia bridge, surgery personally placed by Prof. Dr. Şeker, 4-star hotel accommodation for both visits, VIP airport & clinic transfers, all medications and post-op care, UK aftercare in Glasgow / Surrey / Croydon, and a lifetime implant guarantee. The price is fixed in writing before you commit.
Will my All-on-4 bridge look natural?
Yes — significantly more natural than dentures, and indistinguishable from natural teeth in most everyday situations. The final zirconia bridge is designed individually by our master ceramist with shade, shape, and translucency matched to your face. The temporary bridge during healing is also designed for natural appearance, though slightly less refined than the final restoration.
What happens if an implant fails years later?
If a Straumann Neodent® implant fails at any point in the future, we replace it free of charge under our lifetime guarantee. The four-implant All-on-4 design has built-in redundancy — even if one implant fails after years of use, the bridge often remains stable while we plan replacement. We cover the cost of replacement implant, surgery, new bridge if needed, and the flights and hotel for the return visit.
Why is All-on-4 so much cheaper in Turkey vs the UK?
Same Straumann Neodent® implants. Same surgical protocol. Same FDA-approved materials. The implant in your jaw is identical to what a London Harley Street clinic would place. The price difference reflects clinic operating costs in Antalya — clinic rent, staff salaries, taxation — not implant or clinical quality. UK private dental costs are 3–4× higher than Antalya for the same procedure.
